The four measurements, and why these four
Waist, resting heart rate, and your training pattern feed a published model that estimates VO2max, your body's engine size, without a treadmill in sight. In meta-analyses, every extra MET of that fitness, about 3.5 points of VO2max, was associated with an 11 to 17 percent lower risk of dying from any cause. A resting heart rate 10 beats higher was associated with about 9 percent higher long-term risk.
Push-ups stand in for strength. In a 10-year study of 1,104 working men, those who could do more than 40 had a 96 percent lower rate of heart attacks and other cardiovascular events than those under 10. Women were not in that study, so the calculator uses scaled bands and tells you so.
The one-leg stand is the quiet one. In adults 51 to 75, failing to hold 10 seconds went with markedly worse survival over the following years, and in older adults shorter times are associated with losing independence. It costs nothing to test and it trains fast.

All of this is association evidence from cohort studies and meta-analyses. It tells you where your numbers sit against large populations. It does not diagnose anything, and it is not the "metabolic age" a bathroom scale prints.
